Keep your workplace cleansed of harassment and discrimination complaints. Circulate this form at least bi-annually, and preferably quarterly. Place the executed forms in the employees' personnel files, in a separate file by themselves, or in both. If an employee says that he or she has been the victim of, or witnessed of, a violation, then use the Checklist for Investigating Complaints of Harassment or Discrimination and the Employee Complaint Form to assist in a prompt and thorough investigation.
